Recover from your first mistake

Tell your manager and the people affected when you discover a mistake. State what happened, the impact, and what you have done. Separate confirmed facts from what you still need to check.

Propose a correction. Confirm your authority, the help required, who needs updates, and the deadline. Follow reporting procedures for safety, security, or customer information.

Complete the correction, have it checked, and tell the people involved. Update affected commitments.

Review the mistake with someone who understands the work. Agree on a specific change: “Before sending customer instructions, I will check the version against the approved source.” Ask them to observe your next attempt and give feedback.
